Have you completed your Be Healthy Basics screening?
To be eligible to earn 2018 Wellness Credits, you must be enrolled in the medical plan, get screened, and submit applicable documentation by Sept. 30, 2017.
There are three convenient options to get your Be Healthy Basics screening. Pick the option that is most convenient for you:
- Onsite Employee Health Fairs This is a free, voluntary benefit offered to employees. Get your screening, talk with a health coach, and visit a variety of benefit vendors to get answers to any benefits questions you might have. Click here for Q3 health fair dates.
- Quest Labs Visit a Quest Diagnostics location to get your screening completed. You must take a form with you to get screened and send it into UHC. You can access this form at My.QuestforHealth.com (Registration Key: THR2017).
- Your Doctor Schedule an appointment with your doctor (your doctor must be on the THR medical plan) to get your screening completed. You must take a form with you to get screened and send it into UHC. You can access this form at My.QuestforHealth.com (Registration Key: THR2017).
If your screening results are outside healthy ranges
You can still earn $20 per paycheck in 2018 by completing Rally missions! You may complete three Rally missions (concurrently) on the Rally Rewards site to earn your missing metrics and wellness credits. Each mission lasts 4-5 weeks and must be completed by Sept. 30, 2017. The link to Rally is available on MyTHR.org. (Note: You must get screened before beginning missions in order to earn wellness credits)
Missions are short term goals that will help you improve your health. From drinking enough water each day to getting plenty of sleep each night, Rally gives you Missions you can achieve. Some of the Missions might even surprise you, such as doing something creative each day and laughing or calling friends more often.
If you can’t complete a screening due to medical reasons (example: pregnancy)
If you can’t complete the Be Healthy Basics for medical reasons or have a dispute, visit your physician to complete the Reasonable Alternative Action Form. The form must be completed by your physician and submitted by Sept. 30, 2017.
