72% of Texas Health employees do NOT have a beneficiary named for their 401(k) plan. Are you one of them?

It’s important to name beneficiaries for your Texas Health Resources 401(k) Plan. In the event of your death, benefits would be paid according to plan rules, which might be different from the designation you would choose.

To complete your beneficiary designations online:

  1. Log on to Fidelity NetBenefits® at www.netbenefits.com/thr.
  2. Click the Profile link.
  3. Select Beneficiaries* and follow the online instructions.

Once you have completed your beneficiary designations, you will be able to view them on NetBenefits.

Please be sure to review your choices regularly and update them after certain life events, such as a marriage, divorce, birth of a child, or a death in the family. If you have more than one plan with Fidelity, you will need to set up beneficiaries for each plan. When adding beneficiaries, you can select to add them to all plans or select beneficiaries for each plan individually.

If you do not have Internet access, please call Fidelity at 800-343-0860 to request that a paper beneficiary designation form be mailed to you. Return your completed form to the address on the form. You should allow approximately two weeks for your beneficiary designations to be processed. For questions or assistance, please call Fidelity at 800-343-0860, Monday through Friday, between 7:30 a.m. and 11:00 p.m. Central time.

Note: Beneficiaries for 401(k) are separate from beneficiaries you have designated for other benefits such as Life and AD&D. Each benefit plan must have designated beneficiaries. To update beneficiaries for Life or AD&D, follow these instructions:

  • Click View/Edit Benefits
  • Select Benefits
  • Select the benefit you are naming beneficiaries for (Life or Accident) and select View details
  • Select View/Edit Information
  • Select Beneficiaries and either add or edit your beneficiary information

 

*For online beneficiary designations that require spousal consent, you will be prompted to print out and then return a signed and notarized spousal consent form by mail.
